בופרנורפין(בלועזית:Buprenorphine) הנמכר תחת השמות המסחריים Subutex ,Nopan, Butrans הוא תכשיר אופיאואידי המשמש בשיכוך כאבים חריפים וכרוניים ובטיפול בהתמכרות לתרופות וסמים מקבוצת האופיאטים. הוא ניתן מתחת ללשון, בזריקה, אמצעות מדבקה או שתל בופרנופרין אושר לשימוש רפואי בארצות הברית בשנת 1981. ב-2012 בארצות הברית נרשמו 9,3 מיליון מרשמים לתרופה זו. בופרנורפין נמצא גם בשימוש כסם צורכי הנאה והתרוממות רוח בהזרקה או בהסנפה. בארצות הברית הוא רשום כסם בפיקוח בקטגוריה III. בטיפול בכאבי מירב ההשפעה מושגת תוך שעה ונמשכת כ-24 שעות. בטיפול בהתמכרות לסמים אופיאטיים הטיפול בבופרנורפין מתחיל רק אחרי תחילת תסמיני גמילה וביומיים הראשונים של הטיפול המטופל נמצא בהסתכלות ישירה של עובד בריאות הציבור. בטיפולים לטווח ארוך של ההתמכרות משמשים בנוסחה המשלבת בופרנופרין עם נלוקוסון על מנת למנוע שימוש לרעה על ידי הזרקה.
תופעות לוואי כוללות דיכוי נשימתי (הפחתת הנשימה), ישנוניות, אי ספיקה אדרנלית, הארכת האינטרבל QT ב- ECG, לחץ דם נמוך, תגובות אלרגיות, והתמכרות לאופיאטים. אצל בעלי היסטוריה של פרכוסים, קיים סיכון לפרכוסים.
תופעות תסמונות גמילה אחרי הפסקת בופרונרפין היא הדרך כלל פחות חמורה מאשר אחרי אופיואידים אחרים. הופרנורפין משפיע בצורה שונה על סוגים של קולטנים אופיאואידיים. לפי סוג הקולטן הוא יכול להיות אגוניסטי, אגנויסטי חלקית או אנטאגוניסטי. בעת הריון והנקה השימוש בתרופה אינו מומלץ.
